a.Contributions must be at the following levels to participate

i. First Entry: $156.00
ii. Second Entry: $390.00
iii. Third Entry: $546.00
iv. Fourth Entry: $1,092.00

2. Fill out the appropriate Car Voucher Entries

3. Send your donation, pledge form, and the Car Voucher Entry to:
